胡维勤教授 中医治疗黄体功能障碍性不孕 从20世纪70年代后期开始,运用中药人工周期疗法(简称调周法)调经促排卵治疗不孕,取得了较好的疗效,也形成了治疗不孕症的一大特色。 调周法是在肾藏精、主生殖和女子血海盈亏理论的指导下,根据月经周期中四个时期,即月经期、经后期、经间期、经前期中阴阳消长变化而立治。采用调控肾—天葵—冲任—胞宫生殖轴的方法,拟以滋阴养血-活血化瘀-补肾-活血化瘀的用药思路,促黄体促排卵达到受孕的目的。 黄体功能障碍分为黄体发育不全和黄体萎缩不全两种情况。黄体是月经中期卵巢排卵后产生,黄体期正常为12~16天,如果黄体功能不健全、黄体萎缩,黄体期过短约9~11天或黄体期的天数正常而功能不足,基础体温上升不足0.5℃,孕激素分泌不足,子宫内膜不能产生正常的分泌反应或各部分内膜反应不一致,表现为月经先期或淋漓不净,患者不易受孕或受孕后易流产。如黄体不能按期萎缩退化或不完全退化,持续分泌少量孕激素,使得子宫内膜不能按正常的时间剥落,表现为月经周期正常,但经期延长,可达9~10天甚至更长,量不太多,基础体温双相,常见经期时高相体温才下降。妇科检查在正常范围,内分泌检查提示孕酮促黄体生成激素低于正常值。我们结合妇女表现的基础体温(体温上升缓慢或上升幅度偏低或高温相维持日期短或上升后下降又上升),运用周期疗法治黄体功能障碍性不孕的方法介绍如下: (1)经后期(月经周期的第5~12天):基础体温为低温相。此期精血耗伤,血海空虚,卵泡处于发育阶段,属于阴长期,在肾气作用下逐渐蓄积精血。“经水出诸肾”,肾为经水之源,肾阴为月经来源的物质基础。治法以滋肾益阴养血为主。但阴阳互根,故不忘补阳兼顾肾气。中药选用“滋阴汤”:药用当归12克、醋白芍12克、山药9克、干地黄12克、丹皮9克、茯苓9克、泽泻9克、杞子9克、旱莲草20克、女贞子9克。7帖,水煎服。方中可再加鸡血藤15克、香附10克、丹参12克,以养血调气,佐以补阳之品,也可加入川断9克、菟丝子12克、巴戟天9克。如阴长过快,湿浊气火明显,临床上往往表现为月经先期,量多;治当清心肝、利湿浊,药用苍术、苡仁、山栀、黄柏。如阴不足或阴长过缓,阴阳处于低水平时,基础体温可见上升缓慢或上升幅度偏低,常见月经后期,量少及闭经;治疗补益肾阴7帖,继之加益阳活血之品,如肉苁蓉9克、仙灵脾12克、川芎9克、红花6克、桂枝3克以温阳活血,促进阴阳消长。 (2)经间期(月经第12~16天),基础体温由低温变化至高温相。是月经新时期中至关重要的转化期,阴精盛,重阴转阳,以理气活血为主,配合激发兴奋肾阳之药,使之施泻而以促阴阳转化,促排卵。中药选用当归12克、赤芍9克、丹参12克、川芎9克、泽兰12克、茺蔚子12克、红花6克、川断9克、柴胡9克、香附12克。如阴阳失调,当大补阴精,可按经后期调治;如阴虚盛阳不得以转化,应加强活血通络促排卵药。选用“排卵汤”:当归12克、茺蔚子12克、熟地20克,丹参12克、枸杞子9克、红花6克、香附12克、赤芍9克、泽兰12克。水煎服,每日一剂。基础体温可见高温相维持日期短或上升后下降又上升,偏肾阳虚者加仙灵脾、仙茅各12克,以温补肾阳,小腹痛者加元胡12克,五灵脂(包)、荔枝核各10克,以行气活血止痛。 (3)经前期(月经第17~24天),基础体温为高温相。经前阳长期(黄体期),阴充阳长,以维持阴阳平衡,治宜阴中求阳,温肾暖宫辅以滋肾,采用平补阴阳,气血双调之法,促进黄体功能的正常发挥。选用“促黄体汤”:丹参12克、菟丝子20克、龟板12克,枸杞子9克、旱莲草20克、续断9克、肉苁蓉12克、女贞子9克。水煎服,每日1剂。偏肾阳虚者加淫羊藿12克、仙茅9克,以补肾阳;偏肾阴虚者加熟地15克,山萸肉12克,以滋补肾阴;脾气虚者加黄芪20克,党参12克,以补气健脾;血虚者加熟地15克,当归12克,以养血补血。 (4)月经期(月经第1~4天),月经第25~28天,进入月经期。基础体温急速下降相。重阳转化期,重阳则开,血海满盈而溢下,冲任气血变化急骤,治宜行气活血调经法,促使月经排泄通畅顺利。选用“调经汤”:当归12克、香附12克,熟地12克、赤芍9克、丹参20克、茺蔚子12克、茯苓12克等。水煎服,每日1剂。方中可再加枳壳10克,乌药12克、益母草15克,以助药力。血块多者加五灵脂(包)10克,以活血;出血量多者加仙鹤草30克,三七粉(冲)6克,以活血止血。
